Home Based Services Facilitator

Home-Base Support Services are tailored to meet individuals' needs and preferences for supports, without having to leave their family home. Individuals and families work with a service facilitation agency to develop an Individual Service Plan (ISP) in order to tailor the program to their personal needs.

Envision provides service facilitation for individuals who have been approved for the Home Based Program. These individuals are adult residents of Illinois with a developmental disability in need of significant supports; and who have been determined eligible for funding by the State of Illinois. Envision coordinates billing, hours and assure individuals receive the identified services in their service plan.
